The Chebucto Community Health Team offers FREE programs that support healthy lifestyles and overall wellness for those who live, work, or have a family doctor in the communities of Halifax Mainland (Postal codes: B3M, B3N, B3P, B3R, B3S, B3V).

· Low Intensity 10-Week Exercise Program
· Spring Training for Gardeners 4-Week Program (Beginner)
· Personal Wellness Profile (PWP) Yearly Follow-Up
· Timebank Orientation Session – offered by Chebucto Connections
· Nova Scotia Legal Aid support
The Community Health Team also offers WELLNESS NAVIGATION where we work collaboratively with family physicians, other health providers, and community-based services to help link residents with other programs and supports that will help support individuals to be as healthy as possible.
